Klinische Bedeutung
Die Familie der Herpesviren umfasst mehr als 100 verschiedene Virustypen, von denen acht zu Infektionen beim Menschen führen können. Zu diesen sogenannten humanen Herpesviren gehören neben den Herpes-simplex-Viren des Typs 1 und 2 (HSV-1 und -2) das Varizella-Zoster-Virus (VZV), das Epstein-Barr-Virus (EBV), das Cytomegalievirus (CMV), die Humanen Herpesviren 6 und 7 (HHV-6 und -7) sowie das mit dem Kaposi-Sarkom assoziierte Humane Herpesvirus 8 (HHV-8). Trotz starker struktureller Ähnlichkeiten der einzelnen Erreger können Herpesvirus-Infektionen äußerst unterschiedliche Erkrankungen hervorrufen wie z. B. Lippen- oder Genitalherpes, Windpocken, Gürtelrose, Pfeiffersches Drüsenfieber sowie verschiedene Krebserkrankungen. Während der Schwangerschaft können insbesondere Primärinfektionen mit bestimmten Herpesviren zu schweren Schäden oder dem Verlust des Kindes führen. Einen Impfstoff gibt es bislang nur gegen VZV. Nach der Erstinfektion, welche klinisch häufig unbemerkt verläuft, persistieren Herpesviren ein Leben lang latent im Körper und können später, beispielsweise aufgrund eines geschwächten Immunsystems, reaktiviert werden und (erneut) Krankheitssymptome verursachen.
Diagnostik
Die Labordiagnostik von Herpesvirus-Infektionen stützt sich auf direkte und indirekte Nachweismethoden. Direkte Verfahren, die vor allem bei der Diagnose von akuten Infektionen oder bei Immundefizienz einen hohen Stellenwert besitzen, umfassen z. B. Viruskulturen oder PCR-basierte Tests, mit welchen virale Nukleinsäuren nachgewiesen werden. Als Probenmaterial hierfür dienen Bläschenflüssigkeit, Haut- bzw. Schleimhautabstriche, Blut, Liquor oder bronchoalveoläre Lavage. Bei der Bestimmung Herpesvirus-spezifischer Antikörper in Serum, Plasma oder Liquor handelt es sich hingegen um einen indirekten Erregernachweis. Die Antikörperbestimmung ermöglicht nicht nur eine Aussage darüber, welches Virus sich im Körper des Patienten befindet, sondern kann auch bei der Unterscheidung zwischen primären und rekurrierenden Infektionen helfen.
